Texas Do Not Resuscitate Form (also known as DNR)

The State of Texas requires that you have a specific form if you do not want to have "heroic" means to revive you.

There are many cases where a person may already be in poor health and can decide that they do not want to linger in a hospital and would not want to be resuscitated in the event of a medical emergency. There may also be other reasons.   That is a personal choice that the State of Texas allows you.

However, there are stipulations and forms that must be completed and provided to any EMS personnel responding to your medical emergency.  You may have a device and a form.  The form MUST be the one provided by the State of Texas and filled out and signed correctly.

While the decision to fill out and complete a Texas DNR is a personal one, other important documents also need to be completed: the Texas Living Will (Directive to Physicians), a Durable Power of Attorney for Health Care,  a Declaration of Guardian in Event of Later Incapacity or Need of a Guardian, and an Organ Donation form if so desired.

You should be the one to decide who will have a say in your medical matters and other personal and financial matters if you are unable.  Designate some one.  It is your right.  The official State of Texas DNR form may be found below.
